Step 1: Determine if You have an Addiction with a Self Screening Test

Screening for alcohol and other drug abuse can be used to initiate the process of assessment by identifying your possible problems and determining whether you may have an addiction.

You can print the form and fill it out or use a piece of paper to write in your answers for scoring. The scoring directions appear below the screening form. You score the form yourself and only you see the answers. If you scored a 3 or below, it is most likely that you don’t have an addiction. If you scored a 4 or greater you are at higher risk and you may have an addiction, so move on to Step 2.
